SBSD and Mini Quiz

For MyTask, we wanted some clarity on certain fields in SBSD and we also wanted to provide a suggestion related to MiniQuiz creation.
While creating a SBSD based on the task type, we found out about a question type field which has an option
"Numeric Range Questionairre" . Could you please elaborate more on what exactly this Numeric Range is meant by.
Please find attached the PDF that was sent across to us, for your reference.
Also for mini quiz creation,from a user perspective it would be best to do this in a separate screen rather than a pop up,
because if there are around 10 question, look and feel of the pop up would differ.
Another query we would like to know is, if there can be more than one correct answer. If not, it would be best to provide a radio button instead of a check box.This is just a suggestion.

Client will provide a bottom of the range and the top of the range (min possible number and max possible number)
Examples:
- How many packages of milk are staying on the shelf? min:0 max:100
- On the scale from 1 to 10, where 1 is "very bad" and 10 is "very good", how would you rate the level of customer service in this shop? min:1 max:10
- How old was the respondent? min:14 max:80
2)"Mini quiz"Mini quiz has maximum 5 questions.
There is only one correct answer in each question.
P.S. Sudhir, the topic for SBSD and Mini Quiz already exists. Please don't start new "clone" topics, that can substitute existing ones. Let's keep the whole conversation regarding the same theme in the same topic.

In SBSD, could you please let us know what exactly "Automatically start audio recording when start task execution : Yes No" field is being used for??

Audio record can last 2-30 minutes, depending on the task.

Regarding this audio, say when an agent passes the quiz using the MyTask web interface and this is being assigned to him successfully, he can only execute the task using his smartphone alone right??Or can he even use the MyTask website??Could you please elaborate on this?

Regarding the SBSD, we just got the lay out of the SBSD for Mystery Shopping and Survey task types.Can you please provide us the layout for the remaining task types:-
1) Good Maker
2) Video reporter
3) Store audit
Also, for Survey SBSD(latest version), can you please let us know what exactly the "Forms needed field" is for??

"Forms needed" is the amount of times, survey has to be repeated. So, if "forms needed" field is set at "20", the agent has to interview 20 different people.

For a specific location , if agent needs to interview 20 people, can you please let us know, how the output of that survey (for the specific location) need to be displayed in the client interface while viewing the report

I like the idea of showing the overall stats here.
"View all" button should apply to all questions in the questionnaire, not some of them. When client presses it, a pop-up with a slider should appear, where client can watch all questionnaires separately. Please make changes.
And just for the record, there can be more than 30-50 questions in survey questionnaire.

"View All" link was mean for viewing the comments from 20 people as the question type selected for Q4 is of comment type.

It would be rather best to view all the comments in a new screen
Also for client to watch all the 20 forms, that have answered 30 - 50 questions, it would be very time consuming.
One solution is , in order for the client to view all the 20 forms, client can click on the Download questionnaire button

How many questions is ok from UX perspective? Can we make for example a pop-up, if there is a total of <15 questions and a new screen if there is a total of
